Please click here to complete your New Patient forms at least one​ (1)​ business day before your appointment. ​  If you cannot complete the online forms, please plan to arrive 15 minutes before your scheduled appointment time to complete new patient registration.

 

Due to COVID-19, If you experience a cough or fever or have traveled out of the country two weeks before your appointment, please call our office to let us know before your appointment. 

 

We kindly ask you to keep your scheduled appointment time with us. If you cannot keep your appointment, please cancel or reschedule by calling 916-726-1819, Monday - Thursday.

 

Please bring the following to your appointment:

·  A list of questions for Dr. Nguyen

·  All dental insurance cards and a form of picture ID
